Hearing/Vision/Dental Discount Program NSC offers a discount and managed care program through a national network of independent health care providers of vision, dental, and hearing. NSC has an extensive network of providers already in place and they will add to these as needed to assure that the client base is getting the most available providers in their state. This is not an insured program, it is a discount program only. NSC's Dental Discount Plan, provides discounts of 20-30% below the usual and customary fees of the providers for someone outside the network. NSC's Vision Discount Plan, through its network providers, can provide such things as: a) the lowest retail price on eyeglasses for 30 days from the date of purchase of the difference refunded; b) eyeglass frames and lens quality against manufacturer's defects or flaws in workmanship for one full year or the eyewear will be replaced; c) unrestricted frame and lens selection, including designer, fashion, and traditional frames, and prescription or non-prescription sunglasses; and d) satisfaction and 20% savings on initial contact lenses. There also is a 20% less charge on the provider's documented usual and customary fee for an eye examination. NSC's Hearing Discount Plan, through its netork providers, can provide free hearing evaluations each year and discounts approximately 30% below the ususal and customary fees for someone outside the network. If the policyholder, employer, elects any of these plans they will then be given to all retirees insured under our Medicare supplement policy. The Dental plan may be selected on its own, but the Hearing and Vision must be selected together. Individual selection or choice of participation in this program is precluded.
